This is a consent form for research participation. It contains important information about this study and what to expect if you decide to participate. Your participation is voluntary.

Children over 8 years old

If you are unsure if you want to participant, you should ask any questions you have before making up your mind. You can think about it and discuss it with your family or friends before you decide. It is okay to say “No” if you don’t want to be in the study. If you say “Yes” you can change your mind and quit being in the study at any time without getting in trouble.

Purpose

This exhibit explores your individualized preferences for many everyday things, including other people, dogs, and paintings. This exhibit is primarily used as an educational experience for visitors to Mindworks. Anonymized data may be used to improve the exhibit experience or for future research.

Time Required

This exhibit experience will take approximately 5-7 minutes to complete. It is self-paced; you may stop at any time.

Risks and Benefits

Your participation in this study does not involve any risk to you beyond that of everyday life. Taking part in this research study may not benefit you personally, but you may learn something interesting about yourself, how humans form preferences. Additionally, your participation may help us learn new things that could help others.

Confidentiality

Your anonymized responses will be saved so that we can display your individualized preferences to you. If you decide to withdraw, data collected up until the point of withdrawal may still be saved. Anonymous information may be used for future research studies or shared with other researchers for future research without your additional informed consent.
